It is estimated that millions of preventable adverse drug events occur each year that result in injury and death. According to the Institute of Medicine, millions of people worldwide have difficulty understanding and acting on health information. Furthermore, it is estimated that individuals over 65 years consume more than 30% of all prescription medicines to manage one or more disorders. Of these patients, approximately 50% take 5 or more medications regularly and 12% take at least 10 medications regularly.
It is against this backdrop that Pharmacare Medication Therapy Management Services, a duly registered health service delivery company in Ghana is established to optimize drug therapy, reduce medication problems and improve therapeutic outcomes in patients.
